Academic programs requiring field research or internship experience as part of the course requirement must recall the agreement with the internship location by an “Affiliation” or “Student Field Placement” agreement. The agreement ensures the student`s access to the site and ensures that the site staff monitors the student and provides a useful experience. This model for Professional Services Agreement can be used to obtain services provided to the university by suppliers and consultants. The Professional Services Agreement template contains several sections for entering information about the transactions specific to the services to be provided, including the name of the supplier/advisor, the total contract amount, the duration of the contract, the terms of payment and the scope of work. A workload should be detailed and contain information, including the work/services to be provided/provided by the supplier or consultant; the place where the work or services are performed; milestones; reports, benefits, benefit schedules; finished products; etc. If this model professional services agreement is used for service contracts, a legal review of the professional services agreement by the Office of General Counsel is not necessary unless a change in the language of reference is made. . . .
